By clicking the button below, you can download all necessary forms, print them out, and complete them at your convenience. Bring all completed forms with you to your initial visit along with your insurance card and any previous x-ray/MRI imaging or reports pertaining to your condition. These forms can also be faxed to our office at 814-419-8439 prior to your appointment. This will take some of the stress out of your initial visit and lessen your appointment time as well!
